Connection Planning

Drawing by Van Gogh: Holding HandsConnection Planning is a brief, evidence-informed behavioral intervention to promote social connection. Similar to Safety Planning, Connection Planning involves identifying opportunities and barriers to social connection and making plans to take action.

A free online training to learn how to provide Connection Planning is available via the Finger Lakes Geriatric Education Center (FLGEC), with funding from a HRSA-funded Geriatric Workforce Enhancement Program (GWEP) grant. The FLGEC hosts FREE, self-paced, education relating to the care of older adults. These courses were designed for both health care professionals and family caregivers and can be accessed through the CollaborNation learning management system. The course is titled, “Connection Planning in Long Term Care” and can be accessed here.
