Overview

Welcome to the Culver Medical Group where both adults and children can get their medical care in one location. All of our Attending Doctors are board certified in Internal Medicine and Pediatrics.

Since we are part of UR Medicine Primary Care, we work closely with the region's top specialists at UR Medicine.

We are also home for the Adult Cystic Fibrosis Program.

Patient & Visitor Information

The Culver Medical Group provides 24-hour telephone coverage for emergencies and urgent problems. If you have an emergency/urgent problem after regular office hours, please call our office at (585) 654-5432. The answering service will ask for information about your medical problem, will take your phone number, and have the doctor call you back.

Vaccinating children is the single most important health intervention we can perform as parents and caregivers.

At the University of Rochester Primary Care Network, we firmly believe that all children and young adults should receive the recommended vaccines according to the schedule published by the Centers for Disease Control and the American Academy of Pediatrics.

The recommended vaccines and their schedule are the results of years and years of scientific study and data gathered on millions of children.

Providers

Appointment Information

We see patients by appointment only. You can schedule an appointment by calling during office hours. If you wish more that one child to be seen, please specify this when you call so enough time can be scheduled for each visit.

We understand that you can't plan when you'll get sick, so we try to keep our appointment times flexible to meet your needs. We will try to schedule a sick appointment on the same day that you call. Sometimes this means you will see someone other than your regular doctor, but our team approach to your health care makes it possible for us to work together to meet your scheduling requirements.

Your cooperation in arriving on time for your appointment helps us stay on schedule. We strive to provide each patient with the highest quality care. In the event that our schedule is delayed, please rest assured that you will also get personalized attention during your appointment.

If you must cancel your appointment for any reason, please call at least 24 hours in advance. That will allow us to offer the appointment to another patient.

First Time Patients

  • Please arrive 15 minutes early from your scheduled appointment to allow for registration and paperwork.
  • To save time, fill out our new patient forms and bring them with you.
  • Before your first appointment, call your previous doctor's office and have your records forwarded to our office. Your new doctor will want to review your past medial history and medication list.
  • If your insurance company requires you to notify them when you change doctors, please call them with the name of your new primary care doctor.

What to Bring

  •  Your medical and drug insurance cards.
  • A list of your current medications (with dosages), including any over-the-counter medications or supplements you have taken in the past six months.
  • Check to see which prescription refills you may need so we can provide them for you at the time of your visit.
  • Any forms that need to be completed.

Regular Checkups for Children

Regular office visits take place at 2 weeks, 2 months, 4 months, 6 months, 9 months 12 months, 15 months, 18 months, 2 years, 2.5 years, and then yearly.

Regular Checkups for Teens and Adults

Your doctor will determine when you need a regular check up as an adolescent and adult. At these appointments your doctor will talk to you about recommended screening tests for your age and risk factors.

Shots

The first shots will be at the 2 month visit. By the time your child is 2 years old, he or she should have 4 DTaP, 3 Polio, 3 Hepatitis B, 4 HIB, 4 Prevnar, 1 Chicken Pox, 1 MMR, and 2 Hepatitis A shots. One shot each of DTaP, Polio, MMR, and Chicken Pox is given to children before they enter school.

Prescription Refills

Before coming to your appointment, please check to see which prescription refills you may need so we can provide them for you at the time of your visit.

We require 3 business days for handling your prescription refill requests.

Always call your pharmacy first when you are out of medication. Since we participate in electronic prescribing the pharmacy will process any refill requests that you have directly through our electronic prescription program.

Emergencies/Hospitalizations

If you are experiencing a life-threatening emergency, call 911 or go to the nearest emergency roo. We admit adult patients to Highland Hospital, an affiliate of the University of Rochester Medical Center. The attending doctors at Culver provide care for all patients who are hospitalized at Highland Hospital. While the doctor caring for you may not be the doctor you normally see in the office we communicate regularly with that doctor to provide you the best care.

We admit children to Golisano Children's Hospital at Strong Memorial Hospital. The attending doctors also provide care for your children when they are hospitalized at Strong Memorial Hospital. While the doctor caring for you may not be the doctor you normally see in the office we communicate regularly with that doctor to provide you the best care.

If you or your child is admitted to a hospital where we don't have admitting privileges you will be cared for by a hospital doctor who specializes in hospitalized patients. Please ask them to contact the office to get accurate medical records and medication lists so that you get the best possible care.

Billing

Culver Medical Group is part of Highland Hospital. Because of that, you may receive two separate bills for your care–one from Highland Hospital and one from your provider.

That is so because Medicare requires us to file two claims with them each time we see a patient. Medicare Part A covers your visit to this hospital facility, and Medicare Part B covers the professional services of the physician/provider who provides your treatment. Therefore, you may receive two separate bills to satisfy your coinsurance payment and deductible responsibilities.

Co-payments should be made at the time of your visit. You can pay with cash, personal check or credit card and you now have the ability to pay your hospital or physician bill online at Pay a Bill.

You will be billed for any services not covered by your insurance company.

If you have questions regarding your bill, please call the number listed on the bill and the staff will be happy to answer your questions.

Insurance

We accept most types of insurance including Excellus Blue Cross-Blue Shield, Preferred Care, Aetna, Medicare, Medicaid as well as others.

Please notify our office if your insurance coverage changes. Bring your insurance card with you to your appointment; we will need the new information printed on your insurance card.

We are committed to providing excellent health care for everyone. Financial hardships should never stop you from getting the medical care you need. We offer financial assistance to individuals who are unable to pay medical bills.
