Children with Medical Complexity often experience fragmented care. The Pediatric Complex Care Program team will work in partnership with the patient’s primary care provider and specialty care team to support in problem-solving issues, obtaining access to care and preventing the need for emergency room visits when possible. This innovative model of centralized care coordination and expert consultation at the local level for children with Medical Complexity and chronic illness will improve health equity, optimize health outcomes, and decrease healthcare costs.
We utilize telemedicine visits to allow families easy access to the support of a medical provider when needed.

Patients We Serve
Our focus is on patients that are under-resourced and under-represented. We serve children who receive specialty care and:
- Patients who are followed by more than one specialty within the UR Medicine/Golisano Children’s Hospital network
- Patients who have complex, chronic conditions
- Patients who have a history of several inpatient admissions
- Patients who utilize supportive medical equipment (trachs, G-tubes)
- Patients from birth to age 21
