Referrals to Us

URMC network providers may refer patients to our clinic by choosing "Ambulatory Referral to Preventative Medicine" under Department Specialty. Please be sure to select the specific program you're looking for (i.e. tobacco/vaping, nutrition/lifestyle counseling, or the Rochester Diabetes Prevention Program). 

Choose the specific sub-specialty for your patient and add any specific details about the referral in the comment section. Call (585) 602-0720 for assistance.
