Social Work
Our social workers provide services both in the hospital and the community. As a member of the health care team, they are available to you and your family to provide:
- Emotional support and counseling.
- Help in coordinating plans for your care after you leave the hospital. If you need short-term rehabilitation or a longer stay in a skilled nursing facility, your social worker will help with this process. Your social worker can also arrange an evaluation to determine if nursing care in your home is needed.
- Referrals to community resources such as sliding fee-scale aides, family service agencies, geriatric services, transportation services, or community health nurses for home services.
- Help with Medicaid, Medicare, and other insurance concerns you may have.
Social workers are assigned to all nursing units in the hospital. Services are available seven days a week from 8 am to 11 pm.
Your physician or nurse can help contact the social worker assigned to you. There is no charge for these services.
