Services
HealthWorks offers services for both industrial rehab and occupational health, including:
- Pre-employment physicals
- Drug and alcohol screening
- Wellness programs
- Ergonomic job site analysis
- Industrial rehabilitation.
Services are offered on site or off site, depending on your needs.
Industrial Rehab Services
- Functional Capacity Evaluation (FCE) to assess an individual’s physical ability to do a specific activity.
- Job Function Descriptions including possible modifications of any job in accordance with the Americans with Disabilities Act.
- Job Function Testing to evaluate jobs that are causing challenges.
- Work Conditioning to improve musculoskeletal and cardiovascular fitness.
- Transitional Work Program for employees not yet capable of returning to work full-time after an injury.
- Jobsite Analysis including an on-site evaluation of workstation design and essential job functions.
- Physical/Occupational Therapy including treatment for acute and chronic musculoskeletal disorders of the neck, back and extremities as well as cumulative trauma syndromes.
- Education Programs including back safety, safe lifting techniques and cumulative trauma disorder prevention.
Occupational Health Services
- Physical Examinations including pre-employment, Department of Transportation (DOT), Article 19A, Executive Physicals and Fit for Duty/Return to Work Evaluations.
- Injury Management
- Simplified Care Management in which employers can oversee and organize employees' treatment plans.
- Drug and Alcohol Testing including five-panel DOT testing, non-DOT testing, Breath Alcohol Testing, Medical Review Officer (MRO) services and Hair/Urine Specimen Testing.
- Medical Surveillance including respirator clearance examinations, hazardous materials examinations, asbestos and silica clearance.
- Prevention and Education Programs including bloodborne disease education, drug and alcohol supervisor training, cholesterol screens, flu shot clinics and health fair participation.

Registration Form: To be completed for all new visits to HealthWorks.
Medical History Form: To be filled out and brought to scheduled pre-employment physicals.
Respirator Medical Evaluation Questionnaire: OSHA-required respirator questionnaire to be completed prior to wearing a respirator on the job.
Asbestos Exposure Questionnaire: OSHA-required questionnaire for all who are exposed to asbestos above the permissible exposure limit.
