Geriatric Medicine Community-Wide Grand Rounds meet weekly on Wednesdays from September – June and features multidisciplinary geriatric topics by an expert speaker. These presentations are sponsored by the Finger Lakes Geriatric Education Center, the Division of Geriatrics & Aging in the Department of Medicine at the University of Rochester Medical Center, and the University of Rochester Aging Institute.  All are invited to attend - registration is not necessary.

January 8th: Management of Agitation and Aggression in Patients with Dementia

  • Anton Porsteinsson, MD, William B. and Sheila Konar Professor of Psychiatry, Neurology, Neuroscience, and Medicine, University of Rochester; Director, Alzheimer's Disease Care, Research and Education Program (AD-CARE)
  • Agitation in Dementia is a common behavioral disturbance featuring exaggerated motor activity, sometimes with verbal and/or physical aggressiveness that occurs frequently in patients with cognitive decline; incidence is higher in moderate to severe dementia. Prevalence ranges from 30% to 50% in AD, 30% in dementia with Lewy bodies, 40% in frontotemporal dementia, and 40% in vascular dementia. It adversely impacts cognitive performance, functional status, and patients’ quality of life and enhances caregiver distress and is associated with a higher admission rate to assisted living facilities, higher use of medications, long-term hospitalization, and higher mortality.  Accurate diagnosis and treatment is imperative.  We will explore the most recent advances in this space.

January 22nd: SGLT2 Inhibitor Use in Older Adults, the EMPA-Elderly trial

  • Matthew Taylor, MD, Assistant Professor, Department of Medicine Nephrology Division, URMC
  • This talk is a look at the use of SGLT2 inhibitors for older adults, with an emphasis on reviewing a recent safety trial called EMPA-Elderly.  This will include an overview and history of the use of SGLT2 inhibitors used for treatment of CKD with proteinuria in both diabetic and non diabetic patients.  

January 29th: Palliative & End-of-Life Care for Persons with Dementia

  • Alzheimer’s Disease and related dementias are becoming increasingly prevalent, especially with the shifting demographics of an aging population. Research has elucidated the underlying biologic mechanisms and many genetic risk factors but has yielded little by the way of effective therapies that may delay the onset or reverse the disease. Current pharmacological approaches may provide modest symptomatic benefit for some patients, but drug advertising suggests a greater effect in professional and public marketing. This presentation reviews the current state of affairs of managing dementia from a clinician’s perspective including appropriate prescribing for cognitive, behavioral, and psychological symptoms; pain assessment and management; caregiver education and support; and the role of hospice in end-of-life care.

February 5th: Telehealth Integration of Behavioral Health and Psychiatry into Skilled Nursing Facilities

  • Since 2017, the URMC Telepsychiatry team has delivered services into Skilled Nursing Facilities (SNFs) across the state of NY via (one-on-one) psychiatric consultations to residents of SNFs, (one-to-many) Project ECHO clinics targeting staff of SNFs and Nurse Engagement Specialist assisting with non-pharmacologic interventions.  We have connected with over 150 SNFs, providing over 16,000 visits and 160+ ECHO Clinics. Educating the providers and front-line staff at these facilities is a key goal of this program to increase the skill set of those caring for residents with mental and/or behavioral health needs.

February 26th: Systemic Complications of Acute Stroke: A Scientific Statement from the American Heart Association

  • Ann M. Leonhardt Caprio, DNP, Assistant Professor, Clinical Nursing, URMC
  • Systemic complications are common after stroke, affect different organ systems, and have major impact on patient outcomes. This presentation will review an American Heart Association Scientific Statement to elucidate some major systemic complications of stroke and provide suggestions for clinical care.
