Brain Injury Association of America
- Offers support groups and many resources for brain injury survivors in NYS
- The New York State Department of Health (DOH) Traumatic Brain Injury (TBI) waiver program provides services to persons with a TBI. The purpose of the program is to help persons with a TBI live in the community setting of their choice. Medicaid funds the program.
- To enroll in the TBI waiver, you must:
- Be a Medicaid recipient;
- Choose to live in the community rather than in a nursing facility;
- Be between 18 and 64 years old at the time you apply for waiver services
- Have a primary diagnosis of TBI or similar non-degenerative condition that results in deficits similar to a TBI such as stroke, or anoxia (oxygen supply loss)
- Be assessed to need nursing home level of care; and
- Have an approved plan of available services that reflects the services you need in order for you to live safely in the community.

Article 16 clinics
- OPWDD Article 16 Clinics principally serve persons with Intellectual/Developmental Disabilities (I/DD) and their collaterals, either at the main clinic site or at satellite sites.
- Article 16 Clinic services are habilitative in nature, as they improve or limit disabling conditions, disease, or illnesses through the provision of assessments and therapies to persons with I/DD, through an interdisciplinary model of care.
- Services provided in Article 16 Clinics help individuals with I/DD remain in, or move to, the least restrictive setting and make services available that would otherwise be inaccessible.
- This includes maintenance physical therapy for patients who are looking for long term PT maintenance programs for ROM, strength, etc, ex: CP Rochester, Happiness House
Nazareth College Neurological Clinic
- The Neurological Clinic evaluates and treats people whose function has been impacted by neurological conditions such as cerebral palsy, strokes, traumatic brain injury, or spinal cord injuries.
