Community Resources

Today, more than 90% of children with special health care needs survive into adulthood due to advances in medical care and the early intervention strategies provided by pediatric providers. In fact, adult patients with chronic conditions of childhood origin are now outnumbering pediatric patients among some subgroups. Thus, there is an urgent need to provide the same high-quality care for this target population in the adult health care system.

The Division of Transitional Care Medicine offers a variety of services and supports to meet the needs of referring health care providers and community agencies, as well as the individual goals of patients and their families.

The DTCM Team provides comprehensive support to providers and caregivers locally and regionally:

  • phone and in-person consultations
  • educational sessions
  • patient care protocols
  • educational modules

This support helps your patients stay as close to home as possible for their care and increases the knowledge base of the workforce that is keeping patients healthy in their own home settings.
