Thank you for contacting Flaum Eye Institute Refractive Surgery Center for information about the exciting options available in laser vision correction. We are pleased to provide you further information to better acquaint you with our practice and the unique new generation of technology we provide.

We look forward to meeting you. Expect to spend approximately 2 hours with the Refractive Surgery Center staff at your preoperative exam. We will do a thorough examination including a comprehensive eye health evaluation. The cost of this examination is $100, which is applied to the total cost of the surgical procedure. We will dilate your pupils so expect to be light sensitive and slightly blurry for approximately 4 to 6 hours after the exam. You may want to consider bringing a driver with you if you have had difficulty seeing after an eye exam. In most cases, the doctor will be able to tell you that day if laser vision correction is safe for you and which procedure is the most appropriate for your eyes.

If you wear soft contact lenses, please be sure to discontinue wearing your contacts for at least 1 week prior to your evaluation. If you wear toric contact lenses, please discontinue wearing your contacts for 2 weeks. If you wear hard or gas permeable contact lenses, please discontinue wear for at least 6 weeks. If you are uncertain as to how long to keep your contact lenses out, please call us at (585) 273-2020.

Please complete the following registration form and either send it via email or bring it with you on the day of your evaluation: Patient Registration Forms
