Your Health Care Proxy and Advance Directives
Make Informed Decisions About Your Medical Care
Highland Hospital recognizes the right of patients to make informed decisions regarding their medical treatment. We strive to provide the best environment for decision making about present and future care.
New York State residents have the right to appoint a health care agent with a Health Care Proxy document and the right to state one’s desires concerning life-sustaining treatment with advance directives. While the hospital encourages patients to prepare these documents there will be no discrimination based on whether or not a patient has completed such documents. Learn more about Assigning a Health Care Proxy and download the New York Health Care Proxy form.
Please discuss this matter with those close to you and with your doctor. You are also welcome to consult with Highland’s Patient Representative or Chaplain. Both are familiar with advance directive guidelines and can assist you with this process. Ask a hospital staff member to contact them for you.
