Your Electronic Medical Record
Helping you make important decisions regarding your care about your health is key to our partnership with you. One way to do this is to actively participate in managing your health information. MyChart makes this possible. MyChart is the University of Rochester Medical Center’s (URMC) online service that makes your personal health care information available to you anytime, anywhere using a computer or smartphone.
Benefits of Using MyChart
Free of charge and convenient to use, MyChart gives you the ability to view portions of your URMC outpatient health information including your medications, immunizations, allergies and most lab results. MyChart also provides you with a confidential way to send messages to your doctors and nurses, request prescription renewals and even see your past and upcoming appointments. There may be times when your physician may feel an in-person conversation is the best way to update you on certain tests, and therefore may choose not to make a specific lab result available on MyChart.
Your Hospital Discharge
When you leave Strong Memorial Hospital, you can read an electronic version of your Discharge Instructions/After Visit Summary in MyChart. This summary contains a lot of helpful information such as appointments, phone numbers and health care recommendations that you can review at your convenience. Please note that while MyChart includes your hospital summary, it’s main focus is for you to see your outpatient health information.
Signing Up for MyChart
MyChart is available at no cost to patients who are seen at a URMC outpatient office. If you are a patient at Strong and are interested in signing up for MyChart, your first follow-up appointment may be a good time to discuss MyChart with your physician. You can also sign up for MyChart by calling (585) 275-URMC (8762) 8:00 a.m. to 4:30 p.m. weekdays, or by visiting mychart.urmc.rochester.edu and clicking on “Request Activation Code.
