What do social workers do?

A Social Worker’s role, as part of the medical team, is supportive, informational and educational. We help to assess the psychosocial needs of a patient, provide supportive counseling and connect patients to the appropriate community or hospital resources.

How can a social worker help me at Strong Memorial Hospital?

Social Workers are members of the health care team who assist patients and families with the stresses associated with sudden and chronic illness and its impact on everyday life. Social Workers often consult with your medical team and help to facilitate communication with your doctors and nurses as well as coordinate a patient's discharge from the hospital. Social Workers are also knowledgeable about community resources and can refer you to appropriate programs.

How do I get in touch with a social worker?

The Social Work department is open Monday through Friday from 8:30 a.m. to 5:00 p.m. There are on-call Social Workers available for emergencies after business hours. To find out who your social worker is, ask your medical team or contact the social work department at (585) 275-2851. Additionally, you may visit our main Social Work office for further assistance.
