Primary Care Outreach Program

The Primary Care Outreach Program offers information on well-child care and immunizations and is part of the research initiative, Rochester Childhood Immunization Program. Outreach workers are placed in pediatric practices throughout Rochester. The outreach staff sends reminders about appointments, immunizations and anemia and lead screening for children up to age 2. If transportation is an issue, they can often help with transportation to appointments. If your child does not have insurance, they can even help with obtaining and using health insurance. Lastly, the program can often offer referrals to community services for other family needs.
