What is Geriatric Assessment?

Geriatric assessment is an evaluation of the health and functional ability of an older adult. The goal is to help frail adults 65 and older live as independently as possible.

Complete geriatric assessments include interviewing both the patient and patient’s family, along with information from the patient’s primary care physician.

Our clinic offers remote telemedicine consultation, an interdisciplinary team of geriatricians, nurses, neuropsychologists, pharmacists, and care managers.

The team will perform a thorough assessment. From this consultative visit, we make personalized recommendations to patients, families, caregivers, and primary care physicians. This might include referrals to community resources and service agencies in the area.
