Home health care enables your patients to recuperate from surgery, short-term illness or deal with chronic disease in the comfort of their own homes. We work closely with you to provide your patients and their families the care, education, and support needed for them to become independent in their care. Our skilled team of home health professionals will monitor your patient’s physical, social, and emotional needs and make sure to communicate quickly with you should their status worsen or decline.
Certain insurances require a patient to have a home bound status. Home bound is defined as individuals who cannot leave their home regularly without difficulty and without assistance. Services and programs are based on the needs of your patients.

Making a Referral
Making a Referral and Arranging UR Medicine Home Care is Easy! Our intake nurses are here to take your referral and once received the start of care can be initiated usually within 24 hours of the request. To initiate healthcare services from UR Medicine Home Care, simply call (800) 253-4439 to speak with a nurse in our intake department. We will answer your questions, arrange services and clarify insurance coverage. We offer coordinated interdisciplinary services and work in partnership with the primary physician to deliver compassionate, clinically excellent home healthcare.
Home Healthcare Services Questions
Call (585) 274-4255 or speak with one of our Home Care Coordinators at any one of the hospitals. Certain insurances require completion of a Face to Face Encounter Form by you, the physician. You do not need to initiate this process; if a Face to Face form is needed, we will fax you the form.
Accessing Hospice Care Services
Call (585) 787-8315 or (800) 253-4439 and speak with our Intake Nurse, or contact one of our Home Care Coordinators at any one of the hospitals, or fill out our online Hospice Referral Form, and fax us at (585) 671-4326 with a current medication list for your patient. After one of these steps is completed, our triage staff will call you to confirm receipt of referral.
